Registering with the surgery

To register with the surgery:

You do not need proof of address or immigration status, ID or NHS number. The service is designed and run by the NHS, so you can rest assured that your personal information is safe.
The form helps cut the practice administrative workload and makes it easier for you to register.

Paper forms are still available at reception if you need one. The practice operates a personalised list; therefore, each patient will be assigned their own GP. You will usually be seen by your own doctor.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
